In you would like to participate in Symposium activities and your name does not appear in the list of participants then please submit this form as soon as possible in one of the following ways:

Using the button at the bottom of the page, OR
Print it out and fax to Peta McAllister at +44 (0)24 765 23548, OR
E-mail the same information to peta@maths.warwick.ac.uk.

After receiving your request we will add your name to the list of participants. If your name has not appeared there within a week, please send an e-mail to peta@maths.warwick.ac.uk.

If your name does appear on the list of participants then please check the details given there and e-mail peta@maths.warwick.ac.uk with any corrections.

Please note that this is not an accommodation request form. If you would like us to book accommodation for you at Warwick then you should also send in the accommodation form at least 6 weeks before your visit. Information about the accommodation available is given on the same page. Separate arrangements will be made for accommodation for the workshops at Surrey, Leicester and UMIST.
